• 3 Telehealth Appointment Package $699
   This plan includes 3 monthly appointments and must be used within 4 months after activation.  One $20 visit extension can be applied to this package.  The cost of the initial visit ($399) is waved with the purchase of this program. 

  
  
• 6 Telehealth Appointment Package $1,299
   This plan includes 6 monthly appointments and must be used within 7 months after activation.  Two $20 visit extension can be applied to this package.  The cost of the initial visit ($399) is waved with the purchase of this program. 

  
  
• 9 Telehealth Appointment Package $1,899
  This plan includes 9 monthly appointments and must be used within 10 months after activation.  Three $20 visit extension can be applied to this package.  The cost of the initial visit ($399) is waved with the purchase of this program. 

  
  
• New Patient Telehealth $399
  This initial appointment includes a 20 minute follow up phone consultation the following week.  If you choose to select our 3, 6, or 9 month program the visit cost will be applied to the purchase of that program.
